Know Your Customer Form

Pursuant to 47 CFR § 64.6305 (b) , all voice service providers must file certifications to the FCC’s Robocall Mitigation Database providing detailed information regarding their implementation of the STIR/SHAKEN caller ID authentication framework and/or a robocall mitigation program.

Please indicate whether the Company, its principal(s), controlling person(s), or any individual(s) with an ownership interest in the Company are currently under an adverse judgment resulting from a civil lawsuit filed by the Federal Trade Commission or any other law enforcement action initiated by state or federal agencies. This pertains to matters concerning the use of telecommunications networks, encompassing, but not limited to, anything related to VoIP Services or robocalls.

Customer affirms that the Company has not and will not engage in any illegal use of Autelecom’s services, including but not limited to illegal robocalling or number spoofing", and will comply with all relevant U.S. legislation regarding illegal robocalling, number spoofing and any other legal U.S. law applicable to such services.

Customer affirms and certifies that all information and answers to questions herein are complete, true, and correct to the best of signatory’s knowledge and belief.

The signatory below represents and warrants that he/she has full capacity and authority to sign on behalf of the customer.
